An Ottawa doctor at the centre of an infection scare apologized to her patients on Monday as the city's health officials began alerting nearly 7,000 people that there is a slim chance they may have contracted hepatitis B, hepatitis C or HIV.

Ottawa Public Health says an endoscopic clinic operated by Dr. Christiane Farazli is where as many as 6,800 patients may have been exposed to viruses, although the chances of infection were slim.

The west-end clinic is located at 1081 Carling Ave., Suite 606, officials announced Monday afternoon.

OPH officials said that the College of Physicians and Surgeons of Ontario had inspected the clinic and found that "some infection prevention and cleaning protocols were not always followed."

The city's public health department has been investigating the clinic for the past several months after they became aware of "lapses in infection control."

Dr. Isra Levy, Ottawa's Medical Officer of Health, told a press conference on Monday that the procedures at the centre of the scare are endoscopic procedures, such as colonoscopies.

He said those are no longer being conducted at the clinic, which otherwise remains in operation.

"Ottawa Public Health believes there is a very low risk that patients may have been exposed to hepatitis B, hepatitis C or … the HIV virus during the tests," Levy said.

There is less than a one-in-three-billion chance that someone was infected with HIV, less than a one-in-50 million chance of infection with hepatitis C and less than a one-in-one million chance of hepatitis B infection, according to health officials.

"Even though the risk of infection for those who had the procedures related to this issue is very low, it is important to ensure that affected patients were made aware of the situation so that they can consider being tested for these infections if that is what they wish," Levy said.

Registered letters have been sent to approximately 6,800 patients who had an endoscopic procedure done at the clinic between April 2002 and June 2011.

The letters will advise the patients to consider being tested for the infections with a free blood test that can be ordered by their family doctor or through a walk-in clinic.

Levy said those letters will arrive at the intended addresses beginning Tuesday morning. He added that Ottawa Public Health has also been warning doctors to expect a high volume of patients concerned about the incident.

On Saturday, OPH announced there was a problem at a clinic in the area but it did not immediately identify which one. Levy said that was because "the public health system was not ready in this community over the weekend," leaving the clinic to be identified on Monday.

Ottawa Mayor Jim Watson defended OPH's handling of the health scare, saying that it was "new territory" and that there have been "lessons learned."

Dr. Fred Bray, a member of the Ontario Association of Gastroenterology's board of directors, said the College of Physicians and Surgeons of Ontario was given the authority to begin inspecting out-of-hospital clinics two years ago, and that paved the way for the discovery at Farazli's clinic.

According to the College of Physicians and Surgeons of Ontario website, Farazli graduated from the University of Montreal in 1973 and became an internal medicine specialist in 1978.

She issued an apology through Ottawa Public Health.

"I remain committed to the safety and well-being of my patients and am co-operating fully with OPH in providing patients with this notification," Farazli said in a statement.

"I sincerely regret that the issues that were identified in my facility occurred and I apologize for any inconvenience or anxiety that patients may experience upon receiving this notification. I would also like to reassure patients that the issues that led to this notification have been addressed and that I will be available to support them through this process."

Levy said Farazli has been working with Ottawa Public Health throughout the investigation.

It's still unclear what kind of problems the clinic had with its infection control. There is no evidence that anyone has been infected and the chance that anyone has been infected is low.

Ottawa Public Health has also opened a dedicated information hotline. The number is 613-580-2888. Anyone who underwent endoscopy in the facility between April 2002 and June 2011 and who has not received a letter by Oct. 25 should contact the hotline.

I am highly outrage at this.

It has been said before, but the name of this clinic should have been given out yesterday to answer everyones question of 'omg! did I go there!?'

Secondly, I DEMAND this so called Dr. get their liscence taken from them and the clinic shut down. If you cannot properly clean your tools, a basic and common sens ,of all things, you are not fit to be a Doctor.

And finaly, how DARE you say 'I deeply regret ... I will help them thrue this'' NO! you have done enough! Stop your practice if you trully want to help.

I guess I cannnot see how ' I am sorry that you will die from a terminal deases that you caught from a simple check up' will help calm an infected person. That and anyone else who has had an incounter with potential infected people.

Let's hope this is one of those times that luck shines on us all.

This is not related to this patircular topic but I had this doctor do an endoscope on me back in 2001 I was mis-diagnosed with a hiatus hernia and put on medication for it. Two weeks later I had the same pain as I had prior to the endoscope procedure I admitted myself into a hospital and was told I had a pancreastitis caused by gall stones. I had my gall bladder removed Ever since I've had no problems

The information "when the lapse in infection control in this clinic" was known should have been made public as it is now. If the chance is there we must always remember that if someone is infected its 100% for him given even if its a one in a million.

Ottawa's Medical Officer of Health screwed up. He should have told the public immediately!

You don't give out HALF the information, give everyone in the entire population a cause for worry, then hold back on the specifics. I mean cummon, he isn't a fool....well maybe he is given the recent way this was done.

You either hold ALL the info for the same day, or give it all out a few days later. In this case, what difference does it make to anyone if he had held back the announcement of both bits of information till todays press conference? Does holding the info back make them any sicker, or better - no.ANSWER: NONE.

As for the possibility that someone might have been infected, I remember when they originally said the HIV and Mad Cow Disease was almost impossible to transmit to another human except by a specific method. Well history has proven both of those things wrong.

Here's another issue. Most people who go for these tests are proven to be sick by those tests. So up goes the element of chance that the disease could have been passed on. The numbers aren't the same as if the whole population of mostly well people are accounted for. So they can stick their stats the same place a Colonoscopy goes.

As for no evidence anyone has been infected, well, that's a pretty empty statement, considering they are just contacting the pool of possible infectees starting tomorrow.

This incident was VERY BADLY handled by Dr. Levy.

The chances are less than one in a million of getting Hep B, less than one in fifty million of getting Hep C, and less than one in a billion of getting HIV.

Yes, this is inexcuseable, but I think some of the posters here are being a bit dramatic. I'll bet more than half of the 6800 clients had unprotected sex in the last 20 years.... which would increase their chances of getting any of these things more than a hundredfold.

I had a procedure at Dr. Farazli's clinic and I am on the list of patients that have been exposed. I fail to understand how regular mandatory standard tests had not been performed routinely at this facility and why it took 8 years to detect this major problem. Dr. Farazli and the system are both to blame.

man this is crazy, now all working walk in clinics will be watched which should have been in the first place. we walk into these places thinking they have the best interest for us, now this happens. whats gonna stop the next outbreak if any or unthinkable happens. we have the right to know if their medical instruments or equipment is sterilized and clean so we are protected. this place puts the fear, stress and worry in everyone who had gone to this place for health check. the problem is we let our guard down thinking we be safe so now we have to speak up before the doctor sticks ya with anything.

This is a case of dammed if they didn't, dammed if they did. While allot of mellow drama's are going off over 'possible' infections, and equating it to an outbreak of anthrax in the level of response, this, is most certainly not the case. Or are you all suddenly unaware of the thousands of people that visit hospitals, and of both private and public clinics, that die from highly deadly and infectious bacteria on an annual basis after undergoing very small and routine procedures. This reaction from 'inept' senior officials as some have suggested, is a direct result of what us 'inept' population forced at them after the Canadian Blood Bank tragedy, or was it, we learned from it did we not? You got a warning this time instead of a cover up! Again, the lynch mob was waiting for them regardless of what they did or did not do.

Also not surprising that this is the doctor involved - have you seen the ratings on RateMDs.com? Less than stellar!

As patients we have to ask for and insist on proper standards of safety and hygiene. Hard to do when in a stressful situation but necessary.

I understand and share your concern, but inspections at any frequency -- whether annual, 6-month, monthly, weekly, or even daily -- still will not ensure that procedures are being followed. Maybe the procedure was followed this time, but what about next time? Was the actual procedure followed, but just not documented correctly? Lots of questions to be answered before we start suggesting "solutions."

The message he sent without identifying the clinic was not only malice, but served no purpose other than to scare the wits out of people.

What you have got information on May, but you are telling us now. Sorry does not make life. Just fired and put them in jail. it's a crime.Our doctors just want money, money money. You go see them and they spend just 5 to ten minutes becaue there are hundreded on line. Before seeing them, you have to wait 3 months. Meantime you may die. Just terrible system. No accountibilty of our doctors. There are many hundered specialist doctors driving taxi cab here in ottawa. If we train these specialist doctors cab drivers, they will be more qualified, responsible, honest and carefull than we produce here with million bucks.

PAUL- Wake up and grow up! With the obvious statements, something is being done by the College and the Ministry. Who pays for this? The poor patients, it is not a monetary value! Doctors are not above the law and a majority of them work to HELP individuals like yourself. Think of that next time you are seen by one and need their help. Like any profession, there are bad apples and this happened to be a rotten one. Don't take it out on the rest of the qualified, professional and excellent ones that we do have.

I am getting hysteria developing. It starts in my you no what and moves all around until it comes out my mouth as a scream. 1 in 3 billion is such a huge risk I can't bear it. I just mailed a letter to complain about this and my risk of getting killed in a car accident on the way to the post office is probably near 1 in 2 million. 3 billion compared to 2 million. WOW! reflect on that. I believe when I go to the doctor the chance is probably 1 in 2 (maybe worse) that the doctor is negligent in some way, to my detriment. This does not upset me. But 1 in 3 billion that is such a huge number holy macaroni!

Although the timing and the procedure followed with the release of this information, and the time it took to act upon the issue is clearly deplorable. Please keep in mind, that IF, the follow has been properly completed, then the odds of any kind of infections are pretty low (consider that only a million people live in Ottawa region and the most likely case is 1 in 3 million). There's clear reason to panic, because this is not the first time it's happened, but don't get an anxiety attack either. The healthcare system might need an overall, but pinning problematics with demands only brings about bandaid solutions

Why are there no safeguards in place for these practices? There should be bi-yearly checks by the Public Health Agency - they check restaurants, shouldn't patient care be ahead of that? Its criminal that this clinic was degraded and not practicing safety for almost 10 years before anyone noticed.
This doctor should lose her medical license for incompetency and endangering the lives of her patients!!!!!

Easy for you to say that your sorry. Your not the one worrying about if you contracted HIV or so. Once again, they should all be fired and closed down and leave it to the hospitals. Instead of waiting 5 years, I'll be glad to wait 6 years for a return visit if it means that there is a waiting list at the hospitals now. I can assure you that those clinics are not runned properly with qualified technicians..... or this would have nevered happened.... shame on you!!!

If you deliver news like that you better also say where it took place and who's responsible for it. Can you even imagine how many people were freaked out before they announed it this morning. @ Pat Prachter: Are you kidding? Publicly funded hospitals commit lethal errors, misdiagnose, and struggle with serious infectious issues (that should not have come into existence) all the time. Follow the news! Not having a profit motive, and a mass of insulated unionized administrators and workers running the show, can also create/incubate problems. Your point is mitigated by the realities of healthcare. (By the way, the fact of the matter is that without private-sector involvement, our universal healthcare system would have already collapsed. In Ontario, for example, increasing public funds are even being dedicated to U.S. clinics, to keep the system functioning respectably.)

6800 people may have been put at risk at being infected with Hep C and HIV, but we don't want to tell anyone where this took place until we mail out our letters, since just announcing such information on the media will enable the information to be put out too quickly. Meantime, it's better the entire city feel panic. By delaying advising the general public where this tookm place, it allows people to realize how important health officials are, in time for the next union negotiation

Another example of WHY the privatization of medical services is a bad idea. Private business is run for PROFIT. Publically run hospitals have stringent regulations the must be followed to the letter. Stop the privatization of our medical care. Cutting costs in this area is just stupid.

@ Alma Babic - reread the article they clearly state why they aren't revealing the name of the clinic yet. They want to notify the patients who might have been exposed first.That is the right thing for them to do in this situation.They also said in this article that they will then release the name of the clinic and doctor at that point. The spokesperson even said that they were eager to do so.For those who are comlaining that there have been no criminal charges against the clinic and/or doctor involved, don't rule it out yet. I wouldn't be surprised if it comes next. First and most important though, warn people who may have been exposed to disease.

Why are they not revealing the name of the facility and physician? Protecting the doctor from embarrasment? The public has needs to know.When I asked a endodontist once about wheter they sterilized the equipment he was going to use on me that didn't look particuarly clean he was offended by the question and got a very "cold" treatment after that. It is the Public's right to ask these types of questions and they have a right to know what facilities and doctors can't be trusted.

How the hell do you expect Ottawa residents to feel about this and carry on with their lives? What kind of freakish practice is this? So let's say I and many others received the letter and then what?! Do you really expect to freaking live our lives just like never happened. Unbelievable how you senior officials take this so lightly and expect calm response form the community. Bunch of useless, highly paid individuals with no dignity, moral and conscious.
